Please select the appropriate donor agreements for use in your submission. Donor agreements are important so that history can be shared with the public and researchers can access those materials in the future.
The Deed of Gift form should be used for all non-oral history projects.
All interviews conducted by students, faculty, and/or staff must include the Oral History Release Form.
Any materials you wish to include in the Digital Collections should include a signed Maritime Digital Collections Form.
To adhere to social distancing recommendations, electronic signatures are acceptable. For assistance in e-signatures, read the Adding a Signature document.

Oral histories can be done via audio or video through Zoom, your phone, or computer. If you'd like to do a written, short-answer style history you can use the "Your COVID-19 Story" form.
